Department of Pathology and Microsurgery of Cornea

The Department of Pathology and Microsurgery of Cornea is a leading scientific, medical and diagnostic centre.
Employees of the departmeny have been working on current problems of the corneal transplantation, hereditary, congenital and acquired pathology of cornea and anterior segment of the eye for 85 years.
The Department includes the Division of Microsurgery of Corneal Pathology.
The Division was founded by academician V. Filatov in 1936. At different time the Division was headed by prof. D. Bushmich (1946-1976), prof. T. Gorgiladze (1976-1996).
The department is headed by Galyna Drozhzhyna, DMedSc, professor, Senior Research Scientist, ophthalmologist of higher category.
Basic researches are carried out in the department:
- studying the mutation in TGFBI-gene when hereditary stromal corneal dystrophy;
- studying the role of polymorphic variants of interleukin genes IL18, IL8, IL10 in the development of recurrent corneal erosions when hereditary corneal dystrophy (the department does scientific researches together with the Institute of Molecular Biology and Genetics of National Academy of Sciences of Ukraine);
- studying value of polymorphic variants of genes, that code products of proinflammatory and anti-inflammatory cytokine in hereditary tendency to keratoconus’ development (together with the Institute of Molecular Biology and Genetics of National Academy of Sciences of Ukraine);
- studying of the clinical course of keratitis and corneal ulcers in rheumatoid arthritis depending on the detection of autoantibodies (rheumatoid factor, antibodies to cyclic citrullated peptide) and the level of expression of molecular markers of lymphocyte activation CD 5, CD 25, etc.;
- studying the effectiveness of keratoplasty for optical and therapeutic purposes using various types of transplantation material: keratobioimplant, lyophilized keratobioimplant and amniotic membrane;
- studying particularities of clinical progression and treatment of keratitis caused by a virus of simple herpes, varcella zoster and cytomegalovirus;
- studying peculiarities of inflammatory processes of various etiologies in corneal tissue after amniotic membrane transplantation, cryopreserved at -196°C;
- studying the effective use of blood autoserum in complex conservative treatment of persistent epithelial defects and corneal torpid ulcers of post-infective and neuroparalitical etiology;
- studying of antimicrobial effects in vitro and in vivo in models of fungal keratitis caused by Candida albicans and Fusarium spp. two treatments: 365 nm ultraviolet radiation using a 0.1% riboflavin photosensitizer, and 630-670 nm low-energy laser radiation using a 0.1% methylene blue photosensitizer;
- studying particularities of transformational changes of cornea with the help of confocal microscopy during inflammatory and dystrophic diseases of cornea (keratoconus, corneal dystrophy, keratitis, dry eye disease);
- studying of clinical features of primary and recurrent herpetic keratitis in patients after SARS-CoV-2.

Clinical work on the basis of the department is carried out by the Division of Microsurgical Treatment of Corneal Pathology of the Eye.
Head of the Division:
Viktor Ostashevskyi, Cand. of Med. Sc., ophthalmologist of higher category.
The specialists of the Division have developed and implemented:
- penetrating keratoplasty and its modifies (step-by-step, fungiform, dorsal dipnoous);
- lamellar keratoplasty, interfacial lamellar («biological covering»), peripheral lamellar, interlamellar keratoplasty and others, which are used with optic, medical or cosmetic aim to patients with leukomas and corneal opacity of postinflammatory, posttraumatic and dystrophic ethiology (genetical and acquired);
- reconstructive penetrating keratoplasty when spliced leukomas, that includes synechiotomy, pupil forming, cataract extraction with IOL implantation, frontal vitrectomy, antiglaucomatous operations;
- microsurgery of various kinds of genetically determined corneal ectasia: keratoconus, pellucid marginal degeneration, Terrien’s diseases (penetrating, step-by-step, interlamellar, peripheral keratoplasty, keratectomy with fixing lamellar corneal transplantation);
- accelerated crosslinking of corneal collagen in advanced keratoconus grade I-III;
- microsurgery of acute keratoconus (non-penetrating deep sclerectomy, cryosurgery, intralamellar, interfacial step-by-step keratoplasty);
- microsurgery of cataract with IOL implantation (if indicated with single-step penetrating keratoplasty) – triple procedure;
- cryopreserved amniotic membrane transplantation for the treatment of complicated forms of infectious, neurotrophic and autoimmune keratitis and corneal ulcers;
- conservative and microsurgical treatment of secondary glaucoma;
- microsurgical treatment of recurrent pterygium with peripheral layer keratoplasty for the use of cytostatics (mitomycin-C);
- conservative prevention of inflammatory diseases of cornea and conjunctiva of viral, bacterial, fungal, autoimmune and allergic etiology;
- comprehensive diagnosis, treatment and prevention of herpetic keratitis;
- diagnosis of rare diseases and syndromes (Lyell, Sjögren, scar pemphigoid, Wegener granulomatosis, Stevens-Johnson) with eye involvement using ophthalmic and multidisciplinary research methods;
- photodynamic therapy for the treatment of severe forms of infectious keratitis;
- cryosurgery treatment of diseases of cornea and conjunctiva, also iris’ hydatid;
- complex treatment of severe forms of atopic keratoconjunctivitis with the use of cryodestration of hyperplastic conjunctival tissue;
- complex diagnosis and treatment of dry eye disease using an eye surface analyzer;
- anti-VEGF therapy for corneal neovascularization of various etiologies.
